A finding a delayed ambulance response - played into a girl's death

The Deputy Health and Disability Commissioner has found an ambulance call handler breached the code - in a call over the teenager's asthma attack.
They failed to interpret and enter critical details about her breathing into the software that determines priority for responses.
It meant the seriousness of the girl's condition wasn't fully appreciated, affecting ambulance response.
Responders were immediately sent after a second call, 20 minutes later - but the teen wasn't breathing and died soon after.
