Norman and Gloria Hawkins - who were married for more than 60 years - died after their car crossed the centre line and collided with two other vehicles along State Highway 1 in Chertsey in June 2020.
Two other people were injured.
Coroner Dustin Luo says the airbag control module showed the Hawkins' vehicle braked just zero-point-one seconds before impact.
He says investigations found Norman, who was at the wheel, suffered a medical event which rendered him incapable of controlling the vehicle.
