    The Aged Care Commissioner has found Presbyterian Support Southland breached the rights of an elderly woman in its care in 2021


    The woman had multiple long term conditions, including congestive heart failure - and has since died.

    She was hospitalised after a fall and injury, then moved into the facility's hospital level care.

    Her discharge notes recommended daily weighing - and restricting her fluid intake.

    Commissioner Carolyn Cooper says the service breached rights by not including baseline vital observations.

    She's recommended Presbyterian Support formally apologise to the woman's family.
