    A mid-air mix up between two planes at Wellington Airport in March 2019 meant they had to take evasive action to avoid crashing

    The near miss happened when flight crew on one craft incorrectly identified another plane.

    That put them on a collision course with a third plane.

    A Transport Accident Investigation Commission report found there were sufficient safety measures for preventing a crash ...other issues at play.

    These include flight staff not getting critical information - and procedures potentially meaning an air traffic controller couldn't contact a flight crew.
