    A Northland suicide cluster presents stark revelations of rangatahi being lost in the system


    An inquiry by Northland coroner Tania Tetitaha into the self inflicted deaths of six young people - finds wide ranging barriers to already-fragmented services.

    Children's commissioner Claire Achmad says we need to give focused and ongoing attention to preventing suicide - and mental distress more broadly.

    She says we also need to better connect the systems young people engage with.

    Achmad says that includes putting children at the centre of our education and care, and protection services
