A Health and Disability Commissioner report shows a resident at Elmswood Care Centre had multiple unwitnessed falls.
They weren't adequately documented, and a forearm fracture wasn't promptly assessed.
When her condition deteriorated, there wasn't timely and appropriate intervention, and the woman died 10 days after eventually being taken to hospital.
Aged Care Commissioner Carolyn Cooper says substandard care from staff resulted in multiple shortcomings.
They included issues with monitoring and assessment, documentation and communication.
