Deputy Health and Disability Commissioner Vanessa Caldwell's report shows the woman was rushed to hospital in Auckland's Counties Manukau with stomach pain in 2021.
Open Justice reports low oxygen levels caused a brain injury - after multiple doses of opioids.
Her brother says she no longer has any quality of life.
Caldwell says the hospital had no clear policies to support safely administering opioids, but Health New Zealand argues the woman was assessed appropriately.
