    A toddler was killed after Oranga Tamariki failed to tell a judge about a man's violent past


    The tragic facts of the case are laid out in two coronial findings and a Human Rights Review Tribunal decision today.

    The 17-month-old died 10 days after a judge varied the man's bail, allowing him to live with the boy and his mother in 2015.

    Oranga Tamariki knew the man had a violent past, but told the judge it had no concerns.

    Coroner Marcus Elliott's criticised the agency saying the courts should be able to rely on it to provide full and accurate information.

    The man died in prison while he was on remand accused of murder.
