The Office of the Inspectorate has found many prisoners under 25 aren't offered resources to meet their complex needs.
Amongst its findings, the report says many young people struggle to access rehabilitation opportunities, and don't have suitable accommodation or finances after their sentence.
Chief Inspector Janis Adair says the Department of Corrections is aware of young prisoner needs, but still fails to meet them.
She says she'll be closely monitoring the department's steps to address the challenges.
