    A coroner's issued urgent recommendations - over the death in the Far North of a six-year-old restrained by a centre lap belt


    Joshywn Reihana-Ruka-Wikaira died in 2017 when her mother crashed the car at Omanaia, near Rawene, after taking her eyes off the road to glance at her children.

    Open Justice Reports coroner Ian Telford's ruled the death accidental but has pointed to avoidable elements.

    He says contributing factors to the deadliness of the crash were the mother drinking the night before and her fatigue, and Joshwyn not being in a car seat.

    He's asking the Government to highlight the often-overlooked risks of driving at 8am, the morning after heavy drinking.
