    A coroner has recommended significant changes to Corrections processes around the risk management of offenders


    Paul Tainui - also known as Paul Wilson - raped and murdered 27-year-old Christchurch woman Nicole Tuxford, while on parole for a rape and murder in 1994.

    The news he'd killed again was too much for his first victim's father - who died by suicide.

    Coroner Marcus Elliott's ruled the management of Tainui, contributed to the deaths.

    He's recommended changes to Corrections practices, and the Parole Act, in a bid to avoid similar deaths in future.
