    A doctor's been told to formally apologise to a woman who had surgery without her full consent


    The woman gave permission for her left ovary and fallopian tube to be removed - for a suspected ovarian cyst in her left fallopian tube.

    Her right tube was also removed, which was discussed with her in the holding bay as she was stressed about the operation.

    The woman was told it was unaffected after the surgery, which wasn't the case.

    The deputy Health and Disability Commissioner's recommending an apology, learning modules, and an audit of clinical records.
