    A self-employed midwife, a DHB and a paediatric registrar have been reprimanded over the care of a woman in labour - and of her newborn


    Deputy Health and Disability Commissioner Rose Wall's report outlines the issues after the woman and her midwife arrived at the hospital.

    It was only realised after two hours of pushing, that the woman was only 6cm dilated.

    The baby had to be delivered by caesarean and required resuscitation, before being given the wrong dose of a drug, and dying within 24 hours.

    The DHB has since developed new guidelines, and the midwife has taken steps to meet the Midwifery Council's competency programme.

    The medical professionals involved have been asked to apologise to the family.
