Misha Tremel died in Clevedon, south of Auckland in 2022 after poor risk management as he manually cleared trees.
He was working for a Turoa Logging Limited and Pulley Contracting Limited project , harvesting more than 7000 tonnes of pine.
WorkSafe's found they hadn't properly reassessed their plan - after nearby trees were cut by machinery.
