    A fatal boat capsize off Kaikoura was 'almost certainly' after striking a whale - but a report's found other contributing factors


    The charter boat flipped in Goose Bay almost three years ago - carrying a skipper and 10 Nature Photography Society members.

    Five people died trapped under the vessel and exposed to petrol fumes.

    The Transport Accident Investigation Commission's found emergency response delays and a fuel system defect contributed.

    Chief investigator Naveen Kozhuppakalam says all the victims wore inflated life jackets.

    He says Maritime NZ should educate people on when it's best to remove them, while in the water.
