    Health New Zealand has laid out its road map remedying issues which led to an 11-year-old being mistaken for a mental health patient in her 20s


    The girl was taken to Henry Bennett Metal Health facility in Hamilton in March, and given anti-psychotics before staff realised she was not the missing adult.

    Mental Health and Addiction National Chief Dr Leeanne Fisher will establish a group to action review recommendations between now and the end of October.

    The group will initially meet weekly from May, and make monthly progress reports for a year.

    Actions include rolling out a national unidentified patients policy and a new clinical escalation pathway.
