    A raft of recommendations for Canterbury DHB - after a patient's cancerous lesion went unnoted


    The Health and Disability Commissioner's found the DHB in breach of the consumers' rights code.

    The man died in August 2017 after cancer moved from his lung to his brain - months after DHB staff first noticed it in a CT scan for a spinal injury.

    The Commissioner found doctors failed to record the concerning lesion in the man's discharge notes, or tell his GP.

    He's asked the DHB to create an educational package for staff about expected standards when it comes to incidental findings to stop something like this happening again.

    The DHB's also been asked what changes have been made since the man's death and apologise to his family.
