The woman had multiple long term conditions, including congestive heart failure - and has since died.
She was hospitalised after a fall and injury, then moved into the facility's hospital level care.
Her discharge notes recommended daily weighing - and restricting her fluid intake.
Commissioner Carolyn Cooper says the service breached rights by not including baseline vital observations.
She's recommended Presbyterian Support formally apologise to the woman's family.
