The Transport Accident Investigation Commission's released its findings into the 2015 crash that left seven dead

It revealed the Civil Aviation Authority identified significant non-compliance in the operator's training system and managerial oversight - before the accident.
However, the authority didn't intervene and the operator continued flying.
CAA director Graeme Harris says their oversight of the operator wasn't satisfactory.
He says it certainly points out significant weaknesses in their performance.
