The Transport Accident Investigation Commission findings on a fatal helicopter crash at Fox Glacier in 2015 have revealed concerns about the Civil Aviation Authority.
It hadn't acted - despite finding operators weren't compliant.
The aviation authority has independently reviewed its past surveillance.
Director Graeme Harris says it didn't find any operators posing possible risk - that they hadn't already dealt with.
He says the accident rate's reduced by about 75 per cent in the sector since 2015.
