Those are the findings in a damning report of New Zealand's health system, published in medical journal The Lancet.
It found Maori, Pacific and low-income New Zealanders in particular are losing out and the way the health system is organised is partly to blame.
Co-author Professor Felicity Goodyear-Smith says one of the big issues is a lack of affordable access to primary care- which leads to people remaining unwell and clogging up emergency rooms.
Goodyear says investing in primary care is the most effective way of providing healthcare.
