The Health and Disability Commission has found Oceania Care breached Consumers' Rights in the care of an 87-year-old woman.
It found the care given to the woman over seven months was "unacceptable".
During that time she became increasingly frail, had a non-healing pressure injury and experienced increased pain.
A short time later she was transferred to hospital where she was diagnosed with an ischaemic bowel and died.
Deputy Commissioner Rose Wall says it's concerning that medical attention was not sought when the woman's condition deteriorated.
Wall has recommended the rest home provide a written apology to the woman's family
