Rotorua Lakes Council admits problems with its tree management systems, after a woman died when a tree fell on her car

56-year-old Trish Butterworth died in January last year, after a 23-metre oak split in two during a storm.
At an inquest, a council representative has said Council was too reliant on responding to immediate concerns.
The inquest has heard a 2002 report identified fungal decay in the trunk - but nothing was done, despite a recommendation for a more thorough investigation.
