Commissioners at the Abuse in Care Inquiry are being told of challenges to reporting problems

Human rights lawyers Sonja Cooper and Amanda Hill are giving evidence about civil claims of in psychiatric hospitals and social welfare care.
Sonja Cooper says some complaints weren't dealt with appropriately - or ignored.
She says all too often staff were allowed to resign without referral to police...or even worse, were shifted or promoted to anther institution where the abuse was allowed to continue.
