16-year-old Christchurch boy, Te Hawiki Hona Kiri Te Amo, was visiting his father in Opotiki, when he died after falling from the college's library roof in October last year.
Hawiki and his cousin were kicking the library skylight when it broke... and Hawiki received non-survivable head injuries.
Coroner Gordon Matenga has released his findings today, recommending the college remove the roof skylight or comply with Ministry of Education roofing material guidelines.
He's ordered his findings be distributed to all state-funded schools.
