Censure for Auckland District Health Board - over a young woman's death after developing blood poisoning

The woman was admitted to hospital for postoperative treatment and tests, with an intravenous device in her arm.
The Health and Disability Commissioner's found staff repeatedly didn't document observations of the intravenous site - including swabbing a nearby substance.
Two days after discharge, the woman returned with blood poisoning and died the next day.
Commissioner Anthony Hill found the care was sub-standard, with the DHB not showing reasonable skill and care.
He recommends it implement and review policies in relation to intravenous devices, and consider changes for future cases involving multiple services.
