Rosewood Rest home was overwhelmed by the coronavirus outbreak - which also infected staff.
Documents obtained by the Herald show Canterbury DHB took over Rosewood after concerns about its ability to cope during the pandemic.
A lack of available staff meant the facility breached contractual obligations in multiple areas..
It detailed an apparent lack of contact from Rosewood's owner exasperated health authorities - and the DHB couldn't confirm if it had the required emergency health plan.
More details are expected from the findings of a Ministry of Health review of all rest home clusters - due this week.
