Police have been found to have failed to keep a detainee safe in their care

The Independent Police Conduct Authority has found officers didn't comply with policy -- and neglected their duty of care to a man who attempted suicide in a court cell.
In October 2017, the man was found unconscious after being taken to the Tauranga District Court for a hearing, having tried to asphyxiate himself with his clothing.
He died several days later.
The Authority says officers failed to meet their duty to keep him safe -- by leaving him in the cell with a cut T-shirt.
Police say the failure to follow processes has been raised with the staff involved.
