    A coroner says every hospital in the country can learn from the death of a Dunedin toddler


    Two-year-old Hineihana Mausii died in September 2013 from leukaemia.

    Her mother was sent home twice when she took her to Dunedin Hospital.

    The ODT reports Coroner Brigitte Windley identified numerous failings by health professionals contributed to the girl's death.

    Windley said every DHB should reflect on the adequacy of their ED practices for senior doctors dealing with paediatric patients.
