Trauma for a woman left with a surgical swab inside her abdomen - for four weeks after an operation

The DHB has been found in breach of the Code of Health and Disability Services Consumers' Rights.
Former Health and Disability Commissioner Anthony Hill says it caused the woman in her 40s unnecessary harm, and slowed her recovery.
The report couldn't explain the error - finding during the surgery nurses kept count as required, and no items were unaccounted for.
Hill recommended the DHB order all surgical staff to read the Count Policy and ensure they are up to date with any changes.
He also says they should apologise in a letter to the woman.
