Waikato District Health Board's been censured over the death of a baby

A 28-week pregnant woman collapsed with a ruptured uterus, 17 hours after going to hospital with abdominal pain - when doctors didn't establish a cause.
Her baby died a month later as a result of birth hypoxia.
Deputy Health and Disability Commissioner Rose Wall says an unscarred uterus rupturing in a non-labouring woman is extremely rare - and clinicians would not have considered it.
But she says ineffective communication and co-ordination added to a delay in radiological assessment.
Wall recommended staff-training, including on co-ordination and escalation of care.
