The Transport Accident Investigation Commission's released its report on a water taxi capsize in Stewart Island in 2019 -- leaving several passengers with hypothermia.
TAIC's Harald Hendel says conditions were worse than the skipper expected -- and steep, unpredictable waves flipped the boat.
He says the operator was likely under self-imposed pressure to operate in rough conditions.
Hendel recommended that Maritime Rules be updated -- to ensure all vessels are assessed for stability and buoyancy.
