A rest home's been slated for its care of a dementia patient - who died after becoming unwell

The Deputy Health and Disability Commissioner says staff did not do an adequate clinical assessment on the woman's arrival - or provide a proper care plan for her nutrition.
Toward the end of her stay, staff failed to notice signs of dehydration and deterioration.
The provider's asked to give an update on its revised policies and procedures - and use this case to educate its nurses.
An apology's been issued to the woman's family.
