Parachute life jackets have been deemed unsuitable following a skydiving death in Queenstown

A pair on a commercial tandem skydive crash-landed into Queenstown's Lake Wakatipu in 2018.
The customer was never found, and the tandem master was seriously injured.
The Transport Accident Investigation Commission found the rider's lifejacket didn't properly inflate.
It couldn't determine why - but rules lifejackets certified for parachuting are unsuitable.
