An independent review panel has released its findings on the people - both living alone at the time.
The review concludes missed opportunities contributed to the deaths.
Northern Region Health Coordination Centre Lead Fepulea'i Margie Apa says a key area for improvement is assessing individual welfare needs and wellbeing.
She says it's clear more could have been done.
