Concern killings committed by mentally unwell people in Wellington could have been prevented

The coroner has made recommendations after an inquiry into the killings of five people between 2015 and 2016 in the capital.
Michael Robb identified several flaws in the mental health system.
It includes a lack of resourcing, poor communication between medical professionals and delays in providing appropriate care.
Mental Health Foundation chief executive Shaun Robinson says there were many things that could have been done.
He says that doesn't apply just to the District Health Board system - but across Police, housing and availability of early support.
