    Health New Zealand Waitemata's care of a man fell short of expected standards - at least four times


    A man was admitted to North Shore Hospital feeling light-headed, with chest pain, and shortness of breath.

    He was admitted for 10 days, then discharged without home oxygen.

    After becoming short of breath overnight, he returned - and died the following day.

    Deputy Health and Disability Commissioner Carolyn Cooper says an investigation revealed a lack of clear and correct information during discharge, in particular, a assessment for home oxygen therapy.
